UL Lafayette students selected for verification will be notified. All requested documentation should be submitted to the Office of Financial Aid within 15 days of the request.
Students can also upload their documents via the Financial Aid Secure Document Upload portal. Failure to do so may result in the reduction and/or cancellation of any need-based federal or state aid.
Verification Worksheet
The verification worksheet is used to verify self-reported household size, household number in college, SNAP benefits, and child support paid. Forms needed are listed under the Financial Aid section of ULink or may be printed from our website.
IRS Tax Return Transcript
To request an IRS tax return transcript, call the IRS at 800-908-9946, complete the IRS online request form, or file an IRS Form 4506-EZ by submitting the completed form to the IRS.
