The form must submitted by 11:59 P.M. on the deadline date to withdraw or resign from the University with a grade of W. (See Academic Calendar).
Steps to complete the form:
1.Verify your name, ULID, level, college, major, and classification.
2. Enter a contact phone number.
- Note: This is not a formal update to your phone number.
3. Select "Drop a Class" or "Drop all Classes (Resign)."
4. Enter Reason for Drop by choosing one of the options in the drop drown list.
- Note: If you choose military, you must upload a copy of your military orders. If you choose other, you may utilize the free text box to input any additional information.
5. If dropping a class(es), select the class(es).
- Note: If dropping all classes (resign), skip to Step 6.
