Admission Request Form

Any student, faculty, or staff member wishing to enroll his child in the Child Development Center must complete the waitlist form and your child will be placed on the CDC waiting list. The CDC pledges to assist each family as soon as permissible. Inquiries may be made by mail (Post Office Box 4-3660 Lafayette, LA, 70504) or phone (337-482-5739) or email

Admission Policies

Children of undergraduate students carrying a minimum of nine (9) credit hours or graduate students carrying a minimum of six (6) credit hours that are between the ages of twelve months through six years of age may be admitted into the program as stated by State Law. The CDC provides space for children in compliance with all Rules and Regulations Governing Minimum Requirements for License of all Louisiana Child Care Centers as required by the Louisiana Division of Licensing and Certification-Type III.

Children are enrolled in the CDC in the order in which admission request forms are received. When space is not available, the name of the parent, phone number, and child’s date of birth are secured and placed on a waiting list to be served based on available space. Again priority ranking is always given to students, then faculty and staff.
