To Make a Report

EthicsPoint is NOT intended to replace normal university procedures for reporting and/or dealing with academic dishonesty, student conduct, grade appeals, FERPA (student) privacy issues:

EthicsPoint is NOT a 911 or emergency service:

  • Do not use this site to report events presenting an immediate threat to life or property. Reports submitted through this service may not receive an immediate response. If you require emergency assistance, please call 911 or UL Lafayette Police at 337-482-6447.

Our Commitment

University of Louisiana at Lafayette is an organization with strong values of responsibility and integrity and takes violations of federal, state and university policy seriously.

We are committed to an environment where open, honest communications are the expectation, not the exception. We want you to feel comfortable in approaching your supervisor or management in instances where you believe violations of policies or standards have occurred.

However, in situations where you prefer to place an anonymous, confidential report, we have selected EthicsPoint, a third-party provider, as a means for reporting suspected misconduct. You may report your concerns by calling our hotline at 855-503-8074 or initiating an electronic report by completing the form on the right of this page. This service allows you to report incidents of the following nature:

  • Unethical behavior by faculty or staff
  • Fraud, theft or falsification of documents
  • NCAA compliance
  • Discrimination or harassment
  • Data privacy or data integrity
  • Research or scientific misconduct
  • Environmental or safety risks

The information you provide will be sent to us by EthicsPoint on a totally confidential and anonymous basis, if you should choose. You will receive a report key and password that will allow you to continue to communicate with us and review the status of your report.

This service also is not intended to replace management or supervisory oversight. You may always report your concerns directly to your manager and personnel issues may be addressed to Human Resources.

The University of Louisiana at Lafayette complies with applicable federal and state laws and does not retaliate against those who report suspected wrongdoing or engage in a protected activity.
